The Four Levels Of Hospice
Routine, Respite, General In-Patient and Continuous Care
ROUTINE hospice is provided in the home. This consist of scheduled visits our hospice team, including physician, social worker, nurse aide, chaplain, bereavement coordinator and
volunteer.
Respite Care offers home hospice patients 24-hour care in a hospital or skilled nursing facility for up to 5 consecutive days. The interdisciplinary team maintains scheduled visits with the patient during their hospital care. Respite gives the patient's family a needed break.
General-In-Patient care is done in a hospital or skilled nursing facility when pain and other symptoms cannot be managed in a home setting. The patient returns home once the pain subsides.
Continuous Care is provided to patients to manage acute pain and unmanageable symptoms that is allowed by Medicare. A minimum of 8 hour of care is provided within a 24 hour period.
